Introduction

This form determines if your business is liable for SUTA (State Unemployment Tax Act).

If the employer is determined liable, you will create a login.ok.gov account or log in to an existing login.ok.gov account and you will receive an EzTax login linked to your state account.

If the employer is not liable, you will be informed that the employer is not liable and given the reasons why. You will receive a status determination letter by email. You may choose to request a manual review of the application.

If a liability determination cannot be made, you will be informed that a determination cannot be made and given the reason why. This application will be automatically routed to OESC Tax Staff for review.

What You'll Need

For an optimal experience, we recommend completing this application on a desktop/laptop; however, the application can also be completed on a mobile device.

This application will take approximately 20-30 minutes to complete. Have the following information ready before proceeding:

  • Federal Taxpayer Identification Number (TIN)
  • Employee wage information
  • Primary company contact information
  • IRS 501(c)(3) Determination Letter (if applicable)

To maintain uninterrupted access to the account, we recommend that the person completing this application be one of the following:

  • Business owner
  • Business partner
  • Corporate officer
  • Head of human resources
  • Chief tax officer

In the first part of the application, you will answer questions to determine if you are liable for Oklahoma Unemployment Insurance Tax. The application process will save your progress for up to 10 days to complete the rest of the application. However, if application is not completed within 10 days, you must restart the process.

You will receive your account number immediately after completing the application; however, it may take up to five business days for all features to become available on your account.
